Permanent Supportive Housing Services
Single - Site Permanent Supportive Housing Services
We forge partnerships with property developers to be the on-site provider of supportive services including case management, mental health support, and life skill straining to tenants who were formerly experiencing chronic homelessness. Our dedicated team works closely with each resident to develop personalized plans that address their unique needs, help them achieve their goals, and improve their quality of life.
Scattered -Site Permanent Supportive Housing
Scattered Site - PSH is a flexible housing model where we provide Permanent Supportive Housing (PSH) in various locations throughout LA. This approach allows individuals to live in residential neighborhoods, integrating into the community while still receiving the support they need, having access to case management, healthcare services, and other resources aimed at promoting stability and independence.
Housing Navigation Team
Community Supports is an innovative program connecting housing and healthcare where Housing Works partners with health insurance plans to connect with people who are homeless and needing assisting navigating a complex web of services. Our team physically meets people where they are – whether on the streets, in shelter or elsewhere – and develops a customized housing plan to minimize the duration of each person’s homeless experience.
Community Supports
F.U.S.E
FUSE is a specialized hospital in-reach program in its 11th year of partnership between California Hospital and Housing Works. Our team works with hospital social workers and JWCH Community Health Workers to identify chronically homeless people who frequently use hospital care, targeting them for placement in recuperative care, interim, and permanent housing.
