Housing Works is Service Provider in Housing for Health Program.
Beginning in January 2013, Housing Works Mobile Integrated Service Team will provide services for individuals and families living in 56 units of permanent supportive housing scattered throughout South Los Angeles. Under the Los Angeles County Department of Health Services Housing for Health Program HW was awarded a five year contract to provide services to individuals and families identified as frequent users of DHS emergency and inpatient services as part of the effort to shift use of health services to more appropriate settings while providing access to permanent supportive housing. In partnership with DHS, the blighted properties converted to PSH under the Neighborhood Stabilization Program of the Los Angeles Housing Department will be managed by West Bay Housing Corporation. These newly renovated 56 units will provide permanent housing for folks who have been homeless for 5 to 20 years; many of the units are ADA compliant and will provide a safe environment for people whose disabilities have far too long prevented their independence. HW is proud to be part of this effort to increase PSH and create opportunities for people to thrive in their new homes.

CSH (Corporation for Supportive Housing) awards four teams of organizations who will use supportive housing integrated with care management, and primary and behavioral health to improve health outcomes while reducing public costs among individuals with complex health needs. Housing Works is proud to be part of the Los Angeles team of 38 local housing and healthcare organizations with oversight by Economic Roundtable.
